Independent living options are residential services and support provided to you or your family member in services that live in his/her own home in the community. These settings range from apartments to privately owned homes. This program provides Mentoring Coordinators who offer one-on-one support for a specific number of hours per week. The hours and schedule are based on needs and support level. The services are personalized and designed to support the person in reaching their desired personal outcomes as well as meeting their various needs. Some individuals choose to live by themselves and others choose to live with housemates.

The Family Caregiver Option The Family Caregiver Option is designed for people who wish to live in their own family home or in the home of another family member. In this program, there are three different options for receiving support. The family member may choose to apply with our agency to provide all of the support, our agency enters into a contractual arrangement with the family for delivery of the support services. In addition, our agency provides oversight and monitoring of the family caregiver support in the family home, ensuring the person's satisfaction with their support services and the assurance that all of their needs are being met. Our agency prides itself in assuring that all living and support arrangements best meet the desires and needs of the person receiving support.

Individual Community Connections Program Our Individual Community Connections Program is designed to use the community as a learning environment. All support occurs in the community doing activities of the person's choice. We are committed to people participating in everyday functions of community life while building personal skills and community connections.
